Two things ended up as one
Open the action and swipe the step to the right to pull it out into an action of its own.

It's a machine listening in a moving truck. Here's the fix.
You closed the wrong thing
Tap Undo in the bar at the bottom. If it's gone, the action is in Done: swipe it and tap Reopen. Nothing is deleted by closing.
The time is wrong
Tap the date on the card and set it, or tap Edit and say the right time. The calendar event moves with it.
It misheard a name
Tap the title and type over it. Names of people, pets and businesses are the hardest thing for any phone to hear over noise.
It saved one long action instead of a list
That's the app refusing to lose your words when Apple Intelligence isn't available: everything you said is kept as a single action rather than thrown away. See Permissions and Setup.
Nothing you record is ever discarded because the sorting failed.
