Defects

Trace a problem found later back to the work it came from.

Log a defect

  1. 1Open the verified deliverable the problem came from.
  2. 2Under Defects found later, choose “Log a defect”.
  3. 3Say what's wrong and when it was found.
  4. 4Propose where it came from: “Owner missed the checklist”, “Upstream requirements incomplete” (and which work), or “Review miss”. Choose “Log it”.

A defect links to the signed version it was found in. Anyone on the project can log one, even after the project closes, but not once it's killed.

Confirm and decide

The person who signed the work complete confirms the origin or changes it, with a note. The lead or a co-lead decides: “Reopen for rework” by a date, or record it only, as “Accept as is” or “Fixed downstream”. Reopened work keeps its signed version and signatures; the fix is a new version with new signatures. On a closed project a defect is only recorded, as found after close.

Where they count

Escaped defects are counted by origin in the Gate 6 report and the company report. Defects found after close are shown apart.
